Please find a list a FAQ’s below that we believe will help you complete the online form.

Should I create an account?  You will be prompted to login or create an account.  You should create an account. This allows you to securely save your work and come back at a later time if necessary. You can use your email address or cell phone number.

If you already have an account, you can sign in and complete the form. (You should use the same account to complete forms for multiple children.)

Do I have to answer all the questions? Questions marked with a red asterisk (*) are required.

Do I have to complete the lunch application? No. The lunch application is available for those parents who would like to determine if their children are eligible to receive meal assistance. If you have questions regarding the lunch application, contact Child Nutrition at 662.615.0027.  You can also stop by the Child Nutrition table at Sudduth Elementary School during the centralized registration/residency verification process July 14-15 and July 18-20.

What if I make a mistake? If you would like to make a change, click on the underlined field or click “< Prev” to return to a previous page.

I’ve completed the form, now what? When you have finished entering your information, click “Submit.” This will send all of the information you’ve entered to the school. If you cannot click on this button, you will need to make sure that you have answered all required questions.

What if I have more than one student in the district? Do I need to do this for each child? Yes, because you’ll need to provide information that is specific for each child. We recommend that you submit one and then start another – this will allow you to “snap over” shared family information, which will save you time.

Once I complete the online registration form, do I still need to provide proof of residency documents to my child’s school? Yes.  In addition to online registration, the Starkville Oktibbeha Consolidated School District is implementing centralized residency verification, teacher assignment and schedule pick-up for the 2016-17 school year.

Centralized registration for residency verification, teacher assignments, and student schedule pick-up for all schools/grades will be held July 14 -15 & July 18 – 20 at Sudduth Elementary School.

Parents must provide two proofs of residency verification to complete registration requirements and pick up their child’s schedule/teacher assignments during the centralized registration process.

Acceptable documents to support proof of residency include the following:

    1.  Filed Homestead Exemption Application Form
    2.  Mortgage Documents or Property Deed
    3.  Apartment or Home Lease
    4.  Utility Bill within the past 30 days (only one will be accepted)
    5.  Driver’s License
    6.  Voter Precinct Identification
    7.  Automobile Registration

Do I need to bring any other documents to complete my child’s registration requirements? Parents of new students will also be required to provided a copy of their child’s birth certificate and immunization record (MS Form 121).

My child is entering 7th grade.  Is he/she required to have the Tdap vaccine and do I need to bring an updated immunization form to centralized registration? Yes.  Mississippi now requires a recent Tdap vaccination for all students entering the 7th grade.  This includes new students, current students and transfer students in both public and private schools. Parents of students entering 7th grade are required to provide an updated immunization record.

A free vaccination clinic to administer the Tdap vaccine to rising 7th graders will be offered July 18-20 from 10:00-1:00 at Sudduth Elementary School during centralized registration.

If your child received the TDAP vaccine at AMS in April, you do not need to provide an updated immunization form.  Your child’s updated record will be available at the AMS table during centralized registration.
